Albuquerque/Rio Rancho area
Any one of the Licenses from the below listed:
LCSW or APSW or CHEP or LPC/LPCC, or Licensed Marriage and Family Therapist (LMFT)
Responsibilities:
- Help coordinate care for people with complex health needs.
- Do assessments, create care plans, check progress, and adjust care as needed.
- Support both physical and behavioral health needs.
- Talk regularly with members and their caregivers by phone or in person.
- Use motivational interviewing to support health and behavior changes.
Must-Haves:
- Live in New Mexico.
- Experience in case management or care coordination.
- Experience in behavioral health, managed care, or disease management.
- Bilingual in Spanish.
