About the Role
- The Field Community Connector serves as a community-based advocate and resource for members, utilizing knowledge of the community and available resources to assist vulnerable individuals in managing their healthcare needs.
- This role involves collaborating with the Healthcare Services team to provide non-clinical support in various settings, including homes, nursing homes, shelters, and doctor’s offices.
Responsibilities
- Schedule and conduct in-home visits with members to review care needs and support healthcare goals.
- Act as a trusted community resource by engaging members and connecting them with available services and supports.
- Assist members in understanding and following their care plans in collaboration with Care Managers and healthcare staff.
- Document visits, observations, and member interactions accurately and timely using electronic systems.
- Provide non-clinical field support as the “eyes and ears” for the Healthcare Services team, reporting concerns or barriers to care.
- Support telehealth visits with Care Managers by facilitating technology setup or serving as on-site assistance.
- Maintain regular communication with healthcare staff to ensure continuity of care and follow-up on member needs.
- Identify gaps in service, barriers to care, or social determinants of health (such as food insecurity or housing issues) and assist in addressing them.
Education Qualification
- Must have a valid driver’s license with a good driving record and be able to drive within applicable state or locality with reliable transportation.
Required Skills
- Strong verbal and written communication skills, with the ability to engage compassionately and clearly with members.
- Active listening and empathy to build trust with vulnerable and underserved populations.
- Organizational and time management skills to schedule, conduct, and document multiple home visits.
- Ability to work independently in the field while collaborating effectively with the Care Management team.
- Cultural competency and sensitivity when working with diverse backgrounds and community needs.
- Basic proficiency with mobile devices and MS platforms.
- Bilingual is preferred, but not required (Preferred languages: Bengali, Russian, Chinese, or Spanish).
Required Experience
- Minimum 1 year of experience working with underserved or special needs populations, with varied health, economic, and educational circumstances.
